Lots of people haven't. Nowadays most sites seem to run on php based cms such as wordpress, joomla, etc.

Old timers that can actually hand code a site for you in plain old notepad are a dying breed :)

They don't. But, if you have a site with pages using the same name, but one is htm and the other is html, you'll want to 301 one of those to the other. Usually htm to html, but there are some weird situations where you'll want to do the other way around. :)
