Why they call it "managed servers" this pat of the problem is left up to the hosting business you pay to every month.

What is it that you look for, load averages, ram usage, disk space????
I dont usually upgrade but rather use load balancing methods. You can have a cluster of servers that serve files for you when needed. For example if your server cluster is made up of the following.

Server A
Server B
Server C

If Server A's resources are completely used then the load balancer will use Server B to serve up requests.

You can also set it up so you only have Server A and use a load balancing system that will create Server B and C when needed and then eliminate them once they are no longer needed.

That is for my host to tell me. Why I pay for their equipment and service.

Great point here, sometimes if your paying managed services, I would expect for the host to give you a heads up, some people just don't take this in a good way for some reason. Your host telling you hey bud, its time to cough up some more cash.. hahaha thats probably the opposite of what you would be told, but thats what people think you are trying to say and it becomes a hassle. That said, I think its important to have some general knowledge of how things are supposed to work to have an idea if your sites are being efficient and have enough resources to use on your current set up. knowledge is always good to have.

You can say that again. One time my HD failed and the backup was on the same working drive, I was fucked for months.

If it crashes the host will solve it, otherwise I won't pay them, so I hope they know this shit before it happens.
